Why choose Radiation Oncology? Pros and cons

Why doctors choose it

  • Work-life balance is good, with near-regular hours after PG and very few emergencies.
  • It is a complete specialty, so you can practise immediately after MD or DNB.
  • Rising cancer rates and new cancer centres, including in tier-2 cities, keep jobs available.
  • It combines clinical medicine, physics, computer-based planning and long-term patient care.
  • It gives a strong grounding in oncology for those who later want DM Medical Oncology.

What to weigh carefully

  • The specialty depends on expensive machines and large teams, so setting up your own centre is practically impossible without major investment.
  • Most careers are salaried, and earnings tend to plateau.
  • Acute clinical skills may weaken, because most patients are stable.
  • You depend on referrals from surgeons and physicians.
  • It is viable mainly in cities with cancer centres, not in small towns.

What does a radiation oncologist do?

Radiation oncologists decide when and how radiation should be used for a cancer: before surgery, after surgery or as the main treatment. For external beam therapy, the patient has a CT simulation in the treatment position; the oncologist outlines the tumour and nearby organs, medical physicists design the plan, and the oncologist reviews it before daily treatment on the linear accelerator. Brachytherapy places a radioactive source close to the tumour and is widely used for cervical cancer.

The work also includes concurrent chemotherapy, managing side effects, supportive and palliative care, and long-term follow-up of treated patients. Radiation is used for stereotactic radiosurgery of the brain and body, and for a few non-cancerous conditions. Most patients come through tumour boards and referrals.

Radiation Oncology residency in India: what to expect

Residency is lighter than in Medicine or Surgery, with few night calls, rare emergencies and structured days. Mornings usually begin with ward rounds, followed by the OPD, seminars and follow-up clinics, with procedures such as brachytherapy and CT simulation in the afternoons. Radiation physics and radiobiology are new subjects that need steady study, but they are manageable.

  • Expect some night duties during PG; they reduce sharply afterwards.
  • Gain as much hands-on linear accelerator and 3D planning experience as you can.
  • Attend tumour boards to learn how treatment decisions are made.
  • Build good working relationships with referring surgeons and physicians.

Career options after MD Radiation Oncology

Government service and teaching

Senior residency is required for government assistant professor posts. Faculty positions appear as new colleges and cancer centres open.

Corporate and private hospitals

Private centres usually appoint new graduates as attending consultants for a few years. In tier-2 cities, consultant status can come within about a year.

Own practice

A full radiation centre needs very large investment. Some oncologists run a clinic and treat their patients on machines at partner hospitals.

Super-speciality and fellowships

DM Medical Oncology is the main super-speciality route. Site-specific fellowships at leading cancer centres in India and abroad add further expertise.

Working abroad

Some graduates have built careers abroad after fellowships. Each country has its own registration and training pathway.

Super-speciality and fellowships after MD Radiation Oncology

There is currently no DM in Radiation Oncology itself. Eligibility and seat numbers change every year, so check the current NEET-SS (natboard.edu.in) and INI-SS (aiimsexams.ac.in) bulletins.

DM / DrNB Medical Oncology

3 years. MD or DNB Radiation Oncology has been accepted as a feeder qualification, but eligibility rules have changed in the past, so confirm in the current NEET-SS bulletin.

Site-specific fellowships

Head and neck, breast, gynaecological, paediatric and other site-based fellowships at major cancer centres such as Tata Memorial Centre.

Stereotactic and advanced techniques

Training in SRS, SBRT and brachytherapy at well-equipped centres.

Palliative care

Courses and training in palliative medicine, which is closely linked to oncology.

MD Radiation Oncology vs DNB: which is better?

MD and DNB Radiation Oncology are three-year qualifications. In this specialty, the centre's equipment and teaching matter more than the degree label, so a DNB at a well-equipped centre is better than an MD at a centre with outdated planning systems and weak teaching. Only a couple of diploma seats appear in MCC counselling.

  • Look for a linear accelerator, CT-based 3D planning, brachytherapy and a multidisciplinary tumour board.
  • Hands-on linear accelerator experience strongly influences your first job offer.
  • For DNB, check the institute's exam pass rate.

Radiation oncologist salary in India

Radiation oncologists are mostly salaried. Pay starts modestly in metros and is higher in tier-2 cities, where new cancer centres need consultants. Growth is steady rather than dramatic, and it tends to plateau.

  • Government (verified): Senior Residents at central institutes such as AIIMS are paid at Level 11 of the 7th CPC pay matrix, with an entry basic pay of ₹67,700 per month plus Non-Practising Allowance (NPA) and admissible allowances (as stated in AIIMS recruitment notices). State government pay and stipends vary.
  • Newly qualified, metro (indicative): about ₹1.5–2 lakh per month.
  • After senior residency and one to two years' experience (indicative): about ₹2–3 lakh per month in metros, and more in tier-2 cities.

Indicative only: private figures are based on what practising doctors shared in interviews (2024–25). They are not guaranteed earnings, and actual pay varies widely with city, institution and experience.

Future scope of Radiation Oncology: saturation and AI

Cancer rates are rising, and new cancer centres open every year, including in tier-2 and tier-3 cities, so demand for radiation oncologists continues. Metros show some saturation at senior levels, while entry-level jobs remain available. Linear accelerators are still scarce outside major cities.

Technology is advancing quickly, with stereotactic radiosurgery, image guidance and, gradually, proton therapy. Artificial intelligence already helps with contouring and planning, which is likely to change workflows rather than replace oncologists.

Myths about Radiation Oncology

MythRadiation oncologists have little contact with patients.

FactThey see patients in the OPD and wards, give chemotherapy and follow treated patients for many years.

MythThe specialty needs advanced mathematics.

FactRadiation physics is part of the curriculum, but it is manageable and plays a limited role in daily practice.

MythWorking with radiation makes pregnancy impossible.

FactExposure is monitored and kept within safe limits. Pregnant doctors avoid machine areas and continue with OPD and planning work.

My advice

Should you choose Radiation Oncology? My advice

  • Radiation Oncology gives you a lighter residency, with few night calls and rare emergencies, and a good work-life balance after PG.
  • It is a complete specialty, and DM Medical Oncology remains an option if you want to go further.
  • Remember that it is machine-dependent. Opening your own centre is practically impossible, so this is mainly a salaried career with an earnings plateau.
  • Get hands-on linear accelerator experience. Freshers without it start on lower pay.
  • New cancer centres in tier-2 cities pay more and let you become a consultant sooner.
