Why choose Geriatric Medicine? Pros and cons
Why doctors choose it
- India's elderly population is growing fast, so the patient base will keep expanding.
- Competition is low, because the field is new and specialists are few.
- Academic and leadership roles can come earlier than in more competitive branches.
- Work-life balance is better than in General Medicine.
- Long-term relationships with patients and their families bring gratitude and respect.
What to weigh carefully
- Frailty, dementia and end-of-life care can be emotionally draining.
- Few corporate hospitals have separate geriatrics departments, and public awareness is low.
- Many older patients still go directly to organ specialists or general physicians.
- A private practice may take several years to become established.
- Seats are limited to a few institutes, and Geriatric Medicine has no NEET-SS group of its own.
What does a geriatrician do?
Geriatricians care for older adults who often have several illnesses at once. Their work includes multimorbidity, unusual presentations of common diseases, polypharmacy, falls, frailty, osteoporosis, dementia, Parkinson's disease, stroke, incontinence and chronic pain. The aim is healthy ageing, quality of life and independence, not just the treatment of a single disease.
The work combines medical care with counselling, rehabilitation, nutrition and social support, usually as part of a team. Geriatricians act as the first point of contact for older patients, manage admissions and emergencies, and refer to other specialists when needed.
Geriatric Medicine residency in India: what to expect
Residency resembles internal medicine and covers all body systems, with rotations through departments such as Neurology, Nephrology and Cardiology. Residents manage conditions such as COPD, stroke, heart failure and infections, with exposure to palliative care. Working hours are usually somewhat lighter than in General Medicine.
- Expect mainly OPD and ward work with chronically ill patients.
- Patience, communication and empathy are core skills, so develop them deliberately.
- Learn when to treat actively and when to focus on comfort.
- Build a referral network early.
Verified residents rate their own departments on the HealthPil Residency Index: clinical exposure, teaching, duty hours and toxicity.
Is Geriatric Medicine the right branch for you?
A good fit if you
- Enjoy talking with patients and are patient and empathetic.
- Want an internal-medicine-style specialty with a better work-life balance.
- Are willing to build a practice over time, preferably in a large city.
- Like the idea of being an early entrant in a developing field.
- Value long-term relationships with patients and families.
It may not suit you if you
- Want to become a cardiologist, neurologist or nephrologist.
- Want a shift-based hospital job rather than building a practice.
- Want quick financial returns or an immediate corporate role.
- Would find frailty and end-of-life care very hard to face.

Geriatrician salary in India
Because the specialty is new in India, reliable private earnings figures are not yet available. Government and teaching posts follow standard pay scales, and private practice grows with time and reputation.
- Government (verified): Senior Residents at central institutes such as AIIMS are paid at Level 11 of the 7th CPC pay matrix, with an entry basic pay of ₹67,700 per month plus Non-Practising Allowance (NPA) and admissible allowances (as stated in AIIMS recruitment notices). State government pay and stipends vary.
Private earnings vary widely with city, set-up and experience. Speak to practising geriatricians in the city you plan to work in before deciding.
Future scope of Geriatric Medicine: saturation and AI
India's population is ageing quickly, and the number of people over 60 is expected to rise substantially in the coming decades. Government programmes for elder care are expanding, and more geriatric centres, care homes and hospices are likely to open. Early entrants into this developing field should benefit.
Remote monitoring and digital health tools may support elder care, but the holistic judgement and communication skills of the geriatrician remain central.
