Why choose Palliative Medicine? Pros and cons
Why doctors choose it
- The work is deeply meaningful, bringing comfort and dignity to patients and their families.

- Work-life balance is better than in branches such as Surgery or OBG.

What to weigh carefully
- The work is emotionally demanding, with frequent exposure to death.
- Success is measured in comfort and symptom relief rather than cure.
- It is not free of emergencies, because pain crises can occur at any time.
- It needs genuine commitment to sustain over a whole career.
- Seats are few and offered at a limited number of institutes.
What does a palliative care physician do?
Palliative care specialists manage pain, breathlessness, fatigue, nausea and other distressing symptoms, along with emotional, social, financial and spiritual distress. Their patients include people with advanced cancer as well as heart failure, COPD, liver and kidney disease, neurological conditions and dementia, and children with serious illness.
Care is centred on the patient and family, with shared decision-making and support through end-of-life care and bereavement. The work is team-based, involving nurses, psychologists, counsellors and therapists, and increasingly begins at the time of diagnosis. Interventional procedures such as nerve blocks are used for some types of pain.
Palliative Medicine residency in India: what to expect
At major centres, residency includes busy OPDs, inpatient consultations, emergency cases and home visits. Residents often take patient-care decisions early and explain their reasoning to seniors, which builds confidence. Postings in Medicine, Paediatrics and Oncology broaden clinical knowledge, and learning relies heavily on current research.
- Expect a high volume of patients with complex symptoms.
- Residents may see several deaths in a single shift, so team support matters.
- Read research papers regularly, because practice evolves quickly.
- Build your own support system and interests outside work to manage the emotional load.
Verified residents rate their own departments on the HealthPil Residency Index: clinical exposure, teaching, duty hours and toxicity.

Palliative care physician salary in India
Because the specialty is small and new in India, reliable private earnings figures are not yet available. Government and teaching posts follow standard pay scales.
- Government (verified): Senior Residents at central institutes such as AIIMS are paid at Level 11 of the 7th CPC pay matrix, with an entry basic pay of ₹67,700 per month plus Non-Practising Allowance (NPA) and admissible allowances (as stated in AIIMS recruitment notices). State government pay and stipends vary.
Private earnings vary widely with city, set-up and experience. Speak to practising palliative care physicians before deciding.
Future scope of Palliative Medicine: saturation and AI
Demand will rise as more people live with cancer, heart failure, COPD and liver disease, and as government schemes improve access to care. Very few specialists graduate each year, so early entrants are likely to be in demand, and awareness of the specialty is growing.
Technology may help with symptom tracking and remote follow-up, but palliative care depends on human presence, communication and judgement.
