Why choose Physiology? Pros and cons
Why doctors choose it
- Work-life balance is good, leaving time for hobbies, study or teaching.
- Faculty shortages and new medical colleges create strong demand for physiologists.
- A faculty job is often available soon after MD.
- The work combines teaching, research and, at some centres, clinical services.
- The subject is logical and deepens your understanding of how disease develops.
What to weigh carefully
- It is often seen as non-clinical and less prestigious.
- Income is a fixed salary that rises steadily rather than rapidly.
- In many state colleges, the role is mainly undergraduate teaching.
- Choosing it only for an easy job can lead to frustration.
- Clinical service roles exist mainly at well-equipped institutes.
What does a physiologist do?
In most medical colleges, physiologists teach first-year MBBS students and run practical classes. At central institutes, residents also follow a demanding academic programme of seminars, symposia and journal clubs, and work in laboratories for cognitive function, autonomic function, vascular function and cell biology.
At some centres, physiologists provide clinical services such as intra-operative neuromonitoring, in which they help surgeons protect nerve function during operations, as well as nerve conduction studies, EMG and other functional tests.
Physiology residency in India: what to expect
Hours are predictable, and many residents finish by late afternoon, with weekends largely free. Academic programmes at central institutes are rigorous and intellectually demanding. Residents often discover the full range of career options during residency and senior residency.
- Expect teaching, laboratory rotations and a thesis.
- Use your free time for research and clinical reading.
- Seek exposure to neuromonitoring and functional testing where available.
- Decide your path after some experience as a senior resident.
Verified residents rate their own departments on the HealthPil Residency Index: clinical exposure, teaching, duty hours and toxicity.

- Are curious about how body systems work and enjoy in-depth reading.
- Enjoy teaching and explaining concepts.
- Want a balanced specialty with scope for research.
- Need a stable income early in your career.
- Are interested in neurophysiology, sleep or exercise physiology.
It may not suit you if you
- Do not enjoy the subject itself.
- Are choosing it only because a clinical seat was unavailable.
- Would be troubled by being described as non-clinical.
- Want a high or fast-growing income.
Career options after MD Physiology
Government service and teaching
The academic track runs from senior resident to professor, with strong demand from new medical colleges.
Corporate and private hospitals
Some private hospitals employ physiologists for neuromonitoring, nerve conduction studies, EMG and other functional tests.
Education and entrepreneurship
Coaching, online education and entrepreneurship are options. Clinical practice on the basis of your MBBS may be possible, but it depends on your medical registration and on your employer's rules, as many government and college posts restrict private practice.
Further training and research
Fellowships and courses in sleep, exercise and sports physiology add skills, and PhD and research posts with research organisations are also possible.
Working abroad
Teaching and post-doctoral research roles abroad are possible, depending on each country's requirements.
Super-speciality and fellowships after MD Physiology
Course availability changes every year, so confirm details with the institute before planning.
Physiology has no NEET-SS group. AIIMS Kalyani lists a DM in Clinical Physiology through INI-SS, with very few seats; check the INI-SS prospectus for eligibility.
PhD and post-doctoral research in India or abroad.
Fellowships and courses at selected centres.
Training in neuromonitoring, nerve conduction studies and EMG.

MD Physiology vs DNB: which is better?
MD Physiology is offered in most medical colleges, with few DNB seats and no diploma. For research and clinical service roles, institutes with well-equipped laboratories offer far more.
- Look for laboratories in neurophysiology and autonomic and cardiovascular function.
- Check research activity and support for publications.
- Ask seniors about the teaching load and academic programme.

Physiologist salary in India
Physiologists earn standard medical college salaries, which rise steadily with promotion. Colleges facing faculty shortages sometimes offer higher pay.
- Government (verified): Senior Residents at central institutes such as AIIMS are paid at Level 11 of the 7th CPC pay matrix, with an entry basic pay of ₹67,700 per month plus Non-Practising Allowance (NPA) and admissible allowances (as stated in AIIMS recruitment notices). State government pay and stipends vary.
- Assistant professor (indicative): about ₹1.5–2 lakh per month.
- Associate professor (indicative): about ₹2.5–3 lakh per month.
Indicative only: private figures are based on what practising doctors shared in interviews (2024–25). They are not guaranteed earnings, and actual pay varies widely with city, institution and experience.
Future scope of Physiology: saturation and AI
Faculty demand remains strong because of new medical colleges and shortages in existing ones. Clinical physiology services such as neuromonitoring and sleep studies are growing in larger hospitals.
Technology is expanding research and testing, but teaching, interpretation and research design remain human strengths.

Should you choose Physiology? My advice
- Private colleges have more vacancies but usually pay less than government posts. Faculty posts in good government institutions are hard to secure.
- Tier-2 and tier-3 cities offer openings that many avoid. Pay there is basic, but the work-life balance is excellent.
- In a good academic institution, you get balance, a decent salary and good research opportunities.
- Teaching NEET PG aspirants and medical, dental and physiotherapy students is another path, but only if teaching genuinely interests you.
