Why choose Paediatrics? Pros and cons
Why doctors choose it
- Helping a sick child recover is deeply rewarding for doctors who enjoy working with children.
- Families build lasting trust with their paediatrician, often over many years.
- A clinic is relatively easy and inexpensive to start, supported by steady vaccination and well-baby work.
- Independent practice usually comes sooner than in the surgical branches.
- The number of DM, DrNB and fellowship options has grown considerably.
What to weigh carefully
- It demands great patience. Young children cannot describe their symptoms, and parents are often worried.
- Residency and the early years of practice involve frequent night emergencies.
- Income builds slowly, because parents tend to prefer experienced paediatricians.
- The number of PG seats has risen, and in large cities an MD alone is often no longer enough.
- Critically ill children and difficult conversations with families make the work emotionally intense.
What does a paediatrician do?
A paediatrician diagnoses and treats illness in patients who often cannot explain what is wrong. Small signs carry great weight, drug doses are calculated by body weight, and normal values differ from those in adults. Every consultation effectively involves two people: the child and the parent, who needs a clear explanation, reassurance and, at times, gentle correction of misinformation.
The work includes outpatient clinics, ward and intensive care, the newborn unit, immunisation, growth and development assessment and nutrition advice. Residents learn procedures such as lumbar puncture, central line insertion, intubation and ventilator management. Hospitals with paediatric sub-speciality units offer much deeper exposure.
Paediatrics residency in India: what to expect
Paediatric residency is demanding, especially in high-volume government hospitals, and the first year is usually the hardest. Undergraduate exposure to paediatrics is limited, so the early months are spent learning to recognise which child is truly unwell. A critically ill child may need reassessment every half hour, which is stressful until the child stabilises.
- Expect long duties and frequent night shifts, particularly in the first year.
- Most learning happens at the bedside and in the OPD rather than in lectures.
- Calm, clear communication with children and parents is a core skill, even when you are exhausted.
- Prefer institutes with a high patient volume and dedicated NICU and PICU services.
Verified residents rate their own departments on the HealthPil Residency Index: clinical exposure, teaching, duty hours and toxicity.
Is Paediatrics the right branch for you?
A good fit if you
- Genuinely enjoy being with children and are calm, observant and compassionate.
- Can stay composed with a sick newborn or a deteriorating child.
- Would value long-term relationships with families and, possibly, your own practice.
- Are willing to pursue a fellowship or DM if you plan to work in a large city.
- Are open to tier-2 or tier-3 cities, where an MD alone can still build a strong practice.
It may not suit you if you
- Lose patience easily.
- Want a calm routine and a good work-life balance in your early years.
- Plan to work in a metro but do not want further training after MD.
- Find emotionally charged conversations with families very difficult.
Career options after MD Paediatrics
Government service and teaching
Assistant professor posts are relatively easy to find in newer medical colleges, although leading institutes increasingly prefer candidates with a DM. Senior residency is mainly required for an academic career.
Corporate and private hospitals
Paediatricians with an MD alone often work as registrars or associate consultants for several years before receiving independent admitting rights. A fellowship or DM improves prospects in big-city hospitals.
Own practice
A paediatric clinic is comparatively easy to set up. In smaller towns, a clinic can draw patients from a wide surrounding area, although building trust still takes time.
Super-speciality and fellowships
DM, DrNB and FNB courses in neonatology, cardiology, neurology, critical care, gastroenterology and other areas are increasingly common, particularly among those who plan to work in metros.
Working abroad
Practising abroad requires passing that country's licensing exams. With opportunities in India improving, most paediatricians build their careers here.

MD Paediatrics vs DNB: which is better?
MD and DNB Paediatrics are both three-year qualifications. MD is generally preferred for a teaching career, while for clinical practice a DNB from a busy, well-run hospital can train you as well as, or better than, an MD from a low-volume college. Paediatrics also has a substantial number of two-year diploma seats in MCC counselling, mostly DNB diplomas. Once you are in practice, skill and reputation matter more than the degree.
- Before choosing a DNB centre, check its patient volume, NICU and PICU facilities and past exam results.
- An MD from a high-footfall government hospital provides strong hands-on exposure.
- A diploma is a shorter route into practice. Many diploma holders later take a secondary DNB to obtain a full degree.

Paediatrician salary in India
Early pay in paediatrics varies sharply with region, city size and employer. Big-city salaries for paediatricians with an MD alone tend to be modest at first, while smaller cities and private practice can pay better sooner. A fellowship or DM improves big-city prospects, although teaching posts follow the same pay scale for a given designation.
- Government (verified): Senior Residents at central institutes such as AIIMS are paid at Level 11 of the 7th CPC pay matrix, with an entry basic pay of ₹67,700 per month plus Non-Practising Allowance (NPA) and admissible allowances (as stated in AIIMS recruitment notices). State government pay and stipends vary.
- MD only, metro (indicative): about ₹90,000–1 lakh per month in early roles (figure from South Indian metros).
- MD only, smaller city (indicative): about ₹1.3–1.5 lakh per month.
- Clinic set-up (indicative): about ₹5–6 lakh for a basic clinic.
Indicative only: private figures are based on what practising doctors shared in interviews (2024–25). They are not guaranteed earnings, and actual pay varies widely with city, institution and experience.
Future scope of Paediatrics: saturation and AI
India's large young population will keep the demand for child health care strong, and newborn intensive care continues to expand. At the same time, the increase in PG seats is raising competition, especially in big cities, and paediatrics is moving towards the pattern seen in General Medicine, where many graduates go on to super-specialise.
Technology will improve monitoring and diagnosis, but clinical judgement, communication with parents and procedural skill will remain at the heart of paediatric care.
Myths about Paediatrics
MythPaediatrics is a dead end with nothing after MD.
FactMany paediatricians now pursue DM, DrNB or fellowship training in neonatology, cardiology, neurology, critical care and other areas.
MythPaediatrics is a second choice for those who miss Medicine.
FactIt is a demanding specialty with its own distinct skills, such as examining young children and counselling anxious families.
MythA DM always guarantees higher pay.
FactTeaching posts follow the same pay scale for a given designation, and in some big-city hospitals DM holders still begin as general paediatricians.

Should you choose Paediatrics? My advice
- Paediatrics is rewarding if you truly enjoy children, but residency is physically draining compared with branches such as Radiology.
- Seats have increased, so in tier-1 and tier-2 cities I would plan for a fellowship or DM. In tier-3 cities, an MD alone can still do well.
- Expect a slow start in practice, because parents trust senior paediatricians. Building your name takes time.
- Independent practice is easier to set up than in the surgical branches, and the medico-legal risk is lower than in surgery.
- In the short term, a DM after Medicine usually earns more than a DM after Paediatrics. Choose Paediatrics for the work, not only for income.

What is the salary after MD Paediatrics in India?
In government service, Senior Residents at central institutes such as AIIMS start at ₹67,700 basic pay a month (Level 11 of the 7th CPC pay matrix), plus Non-Practising Allowance and other allowances. In the private sector (indicative), mD only, metro: about ₹90,000–1 lakh per month in early roles (figure from South Indian metros); mD only, smaller city: about ₹1.3–1.5 lakh per month. These figures were shared by practising doctors in 2024–25 and vary with city, institution and experience; they are not guaranteed.
What are the super-speciality options after MD Paediatrics?
The main options are DM / DrNB Neonatology, DM / DrNB Paediatric Cardiology, DM / DrNB Paediatric Neurology, DM / DrNB Paediatric Critical Care, DM / DrNB Paediatric Gastroenterology, Hepatology and Nephrology and Oncology. Seats and eligibility change every year, so check the current NEET-SS, INI-SS and FET bulletins.
Do I need a DM after MD Paediatrics?
Increasingly, yes, for corporate hospitals and leading institutes in big cities. It is not essential for practice in smaller cities or for faculty posts in newer medical colleges.
Is DNB Paediatrics equal to MD?
Both are three-year qualifications. For clinical work, a DNB from a high-volume hospital is well regarded, while an MD keeps an edge for teaching posts.
Is paediatrics saturated?
Competition is rising, especially in big cities, because of the increase in seats. Overall demand remains strong, and smaller cities still offer good opportunities for paediatricians with an MD.
Is a paediatrics diploma worth taking?
A two-year diploma is a quicker route into practice and is widely available through DNB diploma seats. Many diploma holders later complete a secondary DNB. If you plan a teaching or super-speciality career, an MD or DNB is the better choice.
What is the hardest part of paediatric residency?
Usually the first year, with long duties and the challenge of learning to recognise which child is seriously ill.
Is paediatrics a good branch for women?
Yes. Many women choose paediatrics. As with any branch, interest and temperament matter most.
