Why choose PMR? Pros and cons
Why doctors choose it
- It is a clinical specialty that combines medical treatment, interventional procedures and some surgical management.
- Work-life balance is good, with few emergencies.
- Helping a patient walk again or return to independent living is deeply satisfying.
- The job market is not saturated, and new departments are opening in AIIMS institutions and state medical colleges.
- Many patients need long-term care, which suits private practice.
What to weigh carefully
- Awareness is low. Patients, and even some doctors, confuse PMR with physiotherapy.
- You may need to explain your specialty and build referrals from scratch.
- There is no NEET-SS (DM or MCh) route after PMR.
- Some hospitals still rely on physiotherapists instead of employing PMR specialists.
- Early and government pay can be modest, and the range of reading is wide.
What does a PMR specialist do?
A PMR specialist diagnoses, prescribes treatment and leads a team of physiotherapists, occupational therapists, speech therapists, prosthetists and orthotists, rehabilitation nurses and psychologists. Outpatient work covers back and joint pain, osteoarthritis, sports injuries, amputees who need prostheses, and cardiac, pulmonary and cancer rehabilitation.
Inpatient work includes spinal cord injury, stroke, brain injury, cerebral palsy and diabetic foot, with care for spasticity, pressure sores and bladder and bowel function. Procedures include ultrasound-guided injections, nerve blocks, botulinum toxin for spasticity, nerve conduction studies and EMG, and serial casting. Some centres also perform rehabilitation surgery, and PMR specialists play a role in disability certification.
PMR residency in India: what to expect
Residency varies widely by institute. Centres with spinal injury units or surgical rehabilitation can be demanding, with night duties and busy OPDs, while others follow a more regular routine after the first year. The culture is generally supportive, and emergencies are few.
- The first year is usually the busiest, with ward work, referrals and the OPD.
- Learn interventional pain and spasticity procedures and electrodiagnostics thoroughly.
- Exposure depends heavily on the department, so choose carefully.
- Expect to read across many systems, as patients come with varied problems.
Verified residents rate their own departments on the HealthPil Residency Index: clinical exposure, teaching, duty hours and toxicity.
Is PMR the right branch for you?
A good fit if you
- Want a clinical specialty with procedures but regular hours.
- Enjoy variety, reading and long-term relationships with patients.
- Are motivated by restoring function and quality of life.
- Are willing to build your reputation and referrals over time.
- Are open to working away from home, where new departments are opening.
It may not suit you if you
- Want a well-known specialty that needs no explanation.
- Do not want to build referrals and awareness yourself.
- Want a DM or MCh pathway.
- Prefer fast-paced, emergency-based medicine.
Career options after MD PMR
Government service and teaching
New PMR departments in AIIMS institutions and state colleges are creating faculty posts, and some recent graduates are setting up departments themselves. Senior residency is required for faculty posts.
Corporate and private hospitals
Corporate demand is growing as hospitals recognise the value of specialist-led rehabilitation, particularly in metros and in South India.
Own practice
A simple OPD needs little investment, while a full rehabilitation centre with a therapy team needs much more. Long-term patients help a practice grow steadily.
Fellowships
There is no NEET-SS route, but fellowships in pain medicine, neuro-rehabilitation, paediatric rehabilitation and sports medicine add expertise.
Working abroad
PMR is well established in countries such as the United States, the United Kingdom and Australia, each with its own licensing pathway. Check current requirements.
Super-speciality and fellowships after MD PMR
Course names and availability change, so confirm details with the institute before applying. Non-degree fellowships are not NMC or NBEMS degrees.
PMR has no NEET-SS group. Fellowships are the usual route to sub-specialisation. A few AIIMS (Rishikesh, Mangalagiri, Gorakhpur) offer DM Pain Medicine through INI-SS; check the prospectus for whether MD PMR is eligible.
AIIMS New Delhi's one-year Fellowship in Pain Medicine accepts MD PMR after three years of senior residency. Other institutes run their own courses.
Fellowships at selected neuroscience and rehabilitation centres.
Institutional fellowships at selected centres.
Courses and fellowships in India and abroad.
MD PMR vs DNB: which is better?
MD PMR is offered at a limited number of colleges, and DNB at only a few institutes. Both are three-year qualifications. The department's exposure to spinal injury, stroke, paediatric and interventional work matters far more than the label. Diploma seats are almost non-existent.
- Prefer departments with their own OPD, inpatient beds and procedure facilities.
- Check exposure to electrodiagnostics, spasticity management and prosthetics.
- Ask whether a full therapy team works within the department.

PMR specialist salary in India
PMR pay starts modestly in government and bonded posts and grows with skills and reputation. Private hospitals in metros and your own practice can pay well, especially with interventional pain skills.
- Government (verified): Senior Residents at central institutes such as AIIMS are paid at Level 11 of the 7th CPC pay matrix, with an entry basic pay of ₹67,700 per month plus Non-Practising Allowance (NPA) and admissible allowances (as stated in AIIMS recruitment notices). State government pay and stipends vary.
- Private hospitals in metros, first two years (indicative): about ₹2–2.5 lakh per month.
- Established OPD practice (indicative): about ₹3–5 lakh per month.
- Rehabilitation centre in a metro (indicative): an investment of about ₹75 lakh to ₹1 crore, less in smaller cities.
Indicative only: private figures are based on what practising doctors shared in interviews (2024–25). They are not guaranteed earnings, and actual pay varies widely with city, institution and experience.
Future scope of PMR: saturation and AI
Demand will grow with an ageing population, more survivors of stroke and trauma, greater awareness of disability and expanding government rehabilitation programmes. The market is not saturated, and South India is currently ahead of the north in PMR services.
Artificial intelligence may help with gait analysis, outcome prediction and remote monitoring, but rehabilitation remains a human-led team effort.
Myths about PMR
MythPMR is the same as physiotherapy.
FactPMR specialists diagnose, prescribe, perform procedures and lead the rehabilitation team; therapists deliver the prescribed therapy.
MythPMR is only a referral service.
FactMany departments have their own OPD and inpatient beds and manage patients directly.
MythWithout a super-speciality, there is no growth.
FactFellowships in pain, neuro-rehabilitation and other areas, along with private practice, offer strong growth.

Should you choose PMR? My advice
- Pain medicine is the strongest path from PMR. Pain physicians are doing well, and a pain medicine fellowship is open to MD PMR graduates.
- Good jobs in private hospitals are still limited, so expect to build referrals and recognition yourself in the early years.
- There is no NEET-SS route, so plan your skills, especially interventional pain, during residency.
- Choose a department with its own OPD, beds and procedures, because exposure varies greatly between colleges.
