Why choose Psychiatry? Pros and cons
Why doctors choose it
- It offers one of the best work-life balances among clinical branches, with appointment-based work and few emergencies.
- A clinic needs very little investment: a quiet room and basic furniture are enough to begin.
- The unmet need for mental health care in India is very large, and awareness is rising.
- Long-term relationships with patients and families are deeply rewarding.
- Teleconsultation allows flexible ways of working.
What to weigh carefully
- Salaried corporate posts are rare. Most psychiatrists build their own practice.
- A new practice grows slowly in the first few years.
- There are few quick fixes. Chronic illness, relapses and stigma can be frustrating.
- The emotional weight of the work can affect your own wellbeing if you do not look after yourself.
- Misinformation from unqualified voices on social media is widespread.
What does a psychiatrist do?
A psychiatrist assesses, diagnoses and treats mental illness. Day-to-day work centres on careful assessment, medication and psychoeducation, while longer psychotherapy is often shared with clinical psychologists. Common problems include depression, anxiety, sleep disorders, obsessive-compulsive disorder, substance use, technology addiction and burnout.
Much of the work is listening, building trust and educating patients and their families. Chronic conditions such as schizophrenia and bipolar disorder can relapse, much like diabetes or hypertension, so treating one patient often means supporting the whole family. Electroconvulsive therapy (ECT) and brain stimulation techniques such as rTMS are also part of modern practice.
Psychiatry residency in India: what to expect
Psychiatry residency is generally lighter than Medicine, Surgery or OBG, leaving more time for reading and rest at most institutes. Dedicated mental health institutes are busier, with emergencies, closed wards and acutely ill patients. The first six months are an adjustment period, as residents learn to interview, observe and resist the urge to prescribe too quickly.
- Expect some night duties and emergencies, more so at specialised institutes.
- Read the standard textbooks and diagnostic classification systems early.
- Learn from clinical psychologists. Teamwork is central to good psychiatric care.
- Counselling skills needed in private practice are often under-taught, so seek them out.
Verified residents rate their own departments on the HealthPil Residency Index: clinical exposure, teaching, duty hours and toxicity.
Is Psychiatry the right branch for you?
A good fit if you
- Are genuinely interested in human behaviour and the mind.

It may not suit you if you
- Want quick, visible results.
- Become frustrated with patients who take time to open up.
- Need a fixed corporate salary and do not want to run a practice.
- Find long, emotionally heavy conversations draining.
Career options after MD Psychiatry
Government service and teaching
Senior residency posts are relatively easy to find outside the most prestigious institutes, and new medical colleges and AIIMS create faculty positions. An academic career usually requires about three years of senior residency.
Corporate and private hospitals
Fixed-salary hospital posts are uncommon. Many psychiatrists work as visiting consultants at several hospitals, often on a revenue-sharing basis.
Own practice
This is the most common path. A practice typically takes one to three years to build and several more to become well established. Some psychiatrists later add de-addiction or inpatient services, or join group practices.
Super-speciality and fellowships
A DM is not essential at present. Child and adolescent psychiatry adds the clearest value in private practice, and one-year fellowships at leading institutes cover many focused areas.
Working abroad
Countries such as the United Kingdom and Australia have defined pathways for psychiatrists, each with its own registration and examinations. Check current rules before planning.

One-year programmes at NIMHANS and other institutes in areas such as OCD, perinatal psychiatry, brain stimulation, consultation-liaison, forensic and child psychiatry.
Certified courses in CBT and other therapies are useful in practice. They are skill certifications, not degrees.
MD Psychiatry vs DNB: which is better?
MD and DNB Psychiatry are three-year qualifications. MD is generally preferred for teaching posts, and some metro hospitals may initially favour it. In private practice, however, results, communication and word of mouth matter far more than the degree. Only a few diploma (DPM) seats appear in MCC counselling.
- Choose an institute with a good OPD volume and a mix of acute and outpatient cases.
- A DNB centre with poor exam results is a risk, so check past pass rates.
- If you take a diploma, plan for a secondary DNB if you may want a teaching career later.

Psychiatrist salary in India
Psychiatrists usually start modestly, and income grows as patients and families refer others. Smaller cities bring more patients at lower fees, while metros allow higher fees with fewer patients. Earnings in established practice compare well with other clinical branches, usually with shorter working hours.
- Government (verified): Senior Residents at central institutes such as AIIMS are paid at Level 11 of the 7th CPC pay matrix, with an entry basic pay of ₹67,700 per month plus Non-Practising Allowance (NPA) and admissible allowances (as stated in AIIMS recruitment notices). State government pay and stipends vary.
- Private hospitals and clinics, metro (indicative): about ₹2–3 lakh per month.
- Private hospitals and clinics, tier-2 and tier-3 cities (indicative): about ₹3–4 lakh per month.
- Own practice after three to five years (indicative): about ₹3–5 lakh per month, more with added services.
Indicative only: private figures are based on what practising doctors shared in interviews (2024–25). They are not guaranteed earnings, and actual pay varies widely with city, institution and experience.
Future scope of Psychiatry: saturation and AI
India has far fewer psychiatrists than it needs, and demand is growing with rising awareness, technology-related problems and social isolation. Saturation is unlikely in the foreseeable future, and new medical colleges continue to create faculty posts.
Artificial intelligence may help with information and treatment planning, but it is unlikely to replace the human relationship at the heart of psychiatry. Newer treatments, including brain stimulation, continue to expand the field.

- Choose psychiatry only if you are truly interested in it. Patience is the single most important quality.
- It offers one of the best work-life balances among clinical branches, with appointment-based work, teleconsultation and a very low set-up cost.
- Do not expect a fixed corporate salary. Most psychiatrists build their own practice, and it grows slowly at first.
- Psychiatry is not just prescribing; it is about understanding patients and their families. Respect and learn from psychologists.
- Residency is usually lighter than Medicine or Surgery, but dedicated mental health institutes can be busy.
